Adult and Pediatric Medicine
A team of highly-trained professionals provides care in both Adult and Pediatric Medicine. The mix of specialties in Adult Medicine includes Family Practice and Internal Medicine. All physicians are either Board-Certified or Board-eligible in their specialty. Family Nurse Practitioners also provide care to adult patients, and they work closely with FHC’s physicians in coordinating acute episodic medical care, adult immunizations, well-adult exams, medications management and the management of chronic illnesses. Nursing staff assist each provider and provide Triage Services and pre- and post-counseling of patients when required.
Sick- and Well-Child Exams are provided at each Family Health Center location. Age-appropriate childhood immunizations are also provided at each Health Center, including those required for preschool and primary school entry. FHC’s pediatricians are Board-Certified or Board-Eligible in their specialty. They collaborate in the delivery of pediatric care with Nurse Practitioners, who function under clinical protocols. Many parents also find the information published in FHC’s “Pediatrics for Parents” newsletter helpful when caring for their children.

Patient Education |

The FHC Patient Education program offers
information, tools, and support to help patients improve health and prevent
disease. The Health Educator coordinates a comprehensive array of individual
and group programs that focus on child injury
prevention, smoking cessation, weight loss, self-management
of diabetes and other chronic conditions, healthy
pregnancy, and other priority health issues. FHC offers programs for patients
at every stage of life, including children, teens, adults, seniors, expectant
parents, and families.

The
Healthy Ideas (HI!) patient advisory group
has been meeting quarterly since February of 2009 to address health literacy
issues and provide feedback.
Health literacy is the
ability to read, understand, and use health information to
make appropriate health care decisions and follow instructions for treatment.
Nearly half of the U.S. adult population (90 million people)
has difficulty understanding and using health
information, which can have serious negative effects on health. (Institute of
Medicine, Healthy People 2010)
When patients come to Family Health Centers, we give them written forms,
questionnaires, medication instructions, and educational materials. We want
these materials to be easy to understand and useful for our patients. The
HI! Group provides a patient perspective
and important insights for changes to improve our materials. The logo above is
included on all handouts that are approved by the
HI! Group.

Urgent Care |
TOP Urgent Care Services are provided during each Health Center's regular hours of operation. For more serious conditions, the Louisville Metro Emergency Medical Services (EMS) is readily available within minutes to transport patients to a nearby hospital.

Preventive Health Services |

Preventive Health Services are provided by the clinical and health education staff at each Family Health Center. Services include Immunizations, WIC, Nutritional Counseling, Hypertension Monitoring, Well-Child Exams, Family Planning Education, Prenatal Care and Cancer Awareness and Screening services.
FHC’s Preventive Health Services are designed to address each of stage of the human lifecycle: prenatal, pediatrics, adolescent, adult and geriatrics.
Laboratory and Radiology Services |

Diagnostic Laboratory Services are provided at each Family Health Center.

All FHC laboratories are certified under the Clinical Laboratory Improvement Act (CLIA) to perform procedures of “moderate complexity,” and are accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). FHC contracts with reference lab services.
Under written orders of an FHC medical provider, the Radiology Department
performs a full range of diagnostic procedures including chest, thorax, spine,
pelvic and skeletal extremities. All procedures are sent to the University of
Louisville Hospital where a radiologist interprets the exam and provides a
written report to the ordering medical provider.
Women’s Health |
TOP The Centers for Women’s Health at FHC-East Broadway and FHC-Portland offer comprehensive obstetrical and gynecological care to patients throughout the community. Physicians, nurse practitioners, nurses and health educators work together to provide a variety of programs and services from prenatal and perinatal care, to cervical and early breast cancer detection. Obstetric services fit the medical criteria of “low risk” to “high risk.”
Family Planning Classes offer information on the different methods of birth control and sexually transmitted diseases (STDs). Early Prenatal Classes help expectant mothers and their partners learn about the physical and emotional changes that occur during the pregnancy experience, and offer tips on how to cope with them. They also discuss fetal growth and development, healthy eating habits, and the dangers of tobacco, alcohol and drug use during pregnancy.
In the last stage of delivery, Labor & Delivery Classes offer an overview of what to expect during labor, delivery, and the first few weeks the newborn is at home. A six-week Prepared Childbirth Class also helps parents understand what to expect during labor. A Car Seat Safety Class demonstrates the proper way to use a car seat to protect your baby when traveling by automobile.
To promote cancer awareness and early detection, classes are offered on how to properly conduct a Breast Self-Exam and Pelvic Exam. Patients also find useful Women’s Health information in FHC’s “Femme” newsletter.

Healthcare for the Homeless |

Since 1988, FHC has operated the only federally-designated Healthcare for the Homeless Clinic in the Louisville Metro region. Centrally located near downtown Louisville and in close proximity to the city’s homeless shelters, Phoenix Health Center consistently serves some 50-60 homeless adults and children each day.
