Social Services
The aim of Social Services is to help Family Health Center patients remove
barriers to utilizing medical care while improving access to other life
enhancing services.
Services Include:

Medicine Assistance
programs.
Family Health Center patients who cannot pay for prescriptions may be eligible
for help from a variety of programs based on income and other guidelines. Also,
help is available with Medicare Part D applications.

Transportation.

All FHC sites are located directly on bus lines and stops are made at each
facility.
