Services for the Homeless

About

Phoenix Healthcare for the Homeless has served our homeless neighbors since 1988. Centrally located near downtown Louisville and close to the city’s homeless shelters, Phoenix provides services to more than 4,000 homeless adults.

Phoenix provides the following services at our downtown clinical location and through street outreach:

  • Primary Health Care
  • Dental Services
  • Tuberculosis Testing
  • Social Service Outreach, Assessments, and Case Management
  • Mental Health Outreach Services
  • Mental Health Treatment
  • Pharmacy and Medication Assistance
  • Permanent Supportive Housing
  • Respite Care

Respite Care

Family Health Centers’ Respite Care program is for homeless individuals who are being released from the hospital or have another acute medical condition, and need a place to continue their recovery.  Respite patients receive care from FHC’s medical staff, weekly case management to provide direct services, and transportation to and from appointments.

Common Assessment

The Common Assessment team offers homeless outreach, assessment, and support to Louisville’s homeless.

Permanent Supportive Housing

Family Health Centers believes that housing is health care for our homeless neighbors. FHC works to help house homeless individuals, then provide them with peer support, case management and other services to help ensure they are successful in their transition from the streets to a home.
