Family Health Centers welcomes new patients at our seven locations throughout Louisville Metro. All sites provide primary care to adults and have on-site laboratory services. Most offer women’s health services, pediatrics, and counseling services. All Family Health Centers patients may access all services, though travel to another location is sometimes necessary. A video visit is seeing your health care provider on your smart phone or tablet from your home. You will be able to see and talk with your provider about your health concerns, come up with a care plan, and get prescriptions refilled.
All you need is a smart phone to do a Video Visit. When you call to schedule an appointment, let us know you want a video visit. Family Health Centers will send you a secure text message at the time of your appointment.
Immediate Care at Family Health Centers is a walk-in service for adults with urgent health care needs. Patients can walk-in to our immediate care locations for care on a first-come, first-served basis.

A few things to remember for your appointment:
Family Health Centers makes health care affordable for everyone.
Most locations are on a Transit Authority of River City (TARC) bus line. Parking is available at all Family Health Centers sites. Ask about help getting a bus or taxi voucher.
Family Health Centers provides free, trained interpreters for any patient who cannot communicate in English or who is deaf or hard of hearing.
If you need to contact a FHC provider when we are closed, call (502) 774-8631. An answering service will help connect you to a provider on-call. The provider on-call will return your call usually within 30 minutes or less. Your health insurance card may also include an after-hours nurse line for your questions.
